Cerebrospinal fluid neurofilament light predicts longitudinal diagnostic change in patients with psychiatric and neurodegenerative disorders

Objective:
People with neuropsychiatric symptoms often experience delay in accurate diagnosis. Although cerebrospinal fluid neurofilament light (CSF NfL) shows promise in distinguishing neurodegenerative disorders (ND) from psychiatric disorders (PSY), its accuracy in a diagnostically challenging cohort longitudinally is unknown.
Methods:We collected longitudinal diagnostic information (mean = 36 months) from patients assessed at a neuropsychiatry service, categorising diagnoses as ND/mild cognitive impairment/other neurological disorders (ND/MCI/other) and PSY. We pre-specified NfL > 582 pg/mL as indicative of ND/MCI/other.
Results:Diagnostic category changed from initial to final diagnosis for 23% (49/212) of patients. NfL predicted the final diagnostic category for 92% (22/24) of these and predicted final diagnostic category overall (ND/MCI/other vs. PSY) in 88% (187/212), compared to 77% (163/212) with clinical assessment alone.
Conclusions:CSF NfL improved diagnostic accuracy, with potential to have led to earlier, accurate diagnosis in a real-world setting using a pre-specified cut-off, adding weight to translation of NfL into clinical practice.

Background:
Many clinical trials leverage real-world data. Typically, these data are manually abstracted from electronic health records (EHRs) and entered into electronic case report forms (CRFs), a time and labor-intensive process that is also error-prone and may miss information. Automated transfer of data from EHRs to eCRFs has the potential to reduce data abstraction and entry burden as well as improve data quality and safety.
Methods:We conducted a test of automated EHR-to-CRF data transfer for 40 participants in a clinical trial of hospitalized COVID-19 patients. We determined which coordinator-entered data could be automated from the EHR (coverage), and the frequency with which the values from the automated EHR feed and values entered by study personnel for the actual study matched exactly (concordance).
Results:The automated EHR feed populated 10,081/11,952 (84%) coordinator-completed values. For fields where both the automation and study personnel provided data, the values matched exactly 89% of the time. Highest concordance was for daily lab results (94%), which also required the most personnel resources (30 minutes per participant). In a detailed analysis of 196 instances where personnel and automation entered values differed, both a study coordinator and a data analyst agreed that 152 (78%) instances were a result of data entry error.
Conclusions:An automated EHR feed has the potential to significantly decrease study personnel effort while improving the accuracy of CRF data.

Field trials were conducted to determine the effects of glyphosate and/or dicamba simulated drift rates on chipping potatoes (Solanum tuberosum L.) ‘Atlantic’ and ‘Dakota Pearl.’ Sublethal herbicide rates were applied at the tuber initiation stage and consisted of dicamba at 99 g ae ha−1 or glyphosate at 197 g ae ha−1 applied alone or the combinations of dicamba at 20 or 99 g ae ha−1 and glyphosate at 40 or 197 g ae ha−1, respectively. At 7 days after treatment (DAT), the high spray combination of glyphosate plus dicamba resulted in the greatest plant damage (28%). Plant injury from plants treated with the low combination of glyphosate plus dicamba did not differ from the nontreated control. At 21 DAT, visible injury increased to 40% for plants treated with the high combination of glyphosate plus dicamba. Total yield suggested that dicamba and glyphosate caused similar yield reductions as plants that received glyphosate at 197 g ha−1 or dicamba at 99 g ha−1 had lower total yields compared to the nontreated and plants that received the combination of glyphosate (197 g ha−1) and dicamba (99 g ha−1) had lower total yields compared to plants that received either herbicide alone. However, ‘Dakota Pearl’ plants were more sensitive to glyphosate at 197 g ha−1 than were ‘Atlantic’ plants, causing the interaction for most tuber grades. Tuber specific gravity was lower for plants that received glyphosate at 197 g ha−1, dicamba at 99 g ha−1, or this combination, but this reduction would not prevent chip processing. Results reinforce the need for diligence when applying these herbicides in proximity to a susceptible crop, such as chipping potatoes, and the need to thoroughly clean sprayers before application to a sensitive crop.

Purulent pericarditis is a rare diagnosis to be made. It is exceedingly rare as a primary infection. We describe the case of an 18-month-old boy who presented with primary purulent pericarditis and developed a secondary endocarditis. Current literature on the subject is reviewed and discussed.

Introduction
Ellis–van Creveld syndrome is a rare condition associated with a very high incidence of congenital malformations of the heart. Prior reports have suggested increased morbidity and mortality following surgery for congenital malformations of the heart in patients with Ellis–van Creveld syndrome.
Materials and methodsThe Pediatric Health Information System database, an administrative database containing data from 43 free-standing paediatric hospitals in North America, was queried to search for patients with the diagnostic code for Ellis–van Creveld syndrome between 2004 and 2011. Those patients who underwent cardiac procedures were compared with those who did not with respect to measures of healthcare utilisation.
ResultsA total of 138 admissions occurred in 93 patients with Ellis–van Creveld syndrome during the study period. Of these, 74% had an underlying diagnosis of congenital malformations of the heart. Half of the patients in the sample underwent a cardiac surgical or interventional catheterisation procedure. Patients who underwent a cardiac procedure had a longer hospital length of stay, higher incidence of intensive care unit admission, and higher total and per day hospital charges than patients who did not undergo cardiac surgery during admission.
ConclusionsIn a large group of inpatients with Ellis–van Creveld syndrome, the prevalence of congenital malformations of the heart was similar to that reported in prior studies. Cardiac surgical and interventional procedures appear to drive a substantial portion of healthcare utilisation in these patients.

The recording and analysis of a burnt mound and adjacent palaeochannel deposits on the floodplain of the River Soar in Leicestershire revealed that the burnt mound was in use, possibly for a number of different purposes, at the transition from the Neolithic to the Bronze Age. An extensive radiocarbon dating programme indicated that the site was revisited. Human remains from the palaeochannel comprised the remains of three individuals, two of whom pre-dated the burnt mound by several centuries while the partial remains of a third, dating from the Late Bronze Age, provided evidence that this individual had met a violent death. These finds, along with animal bones dating to the Iron Age, and the remains of a bridge from the early medieval period, suggest that people were drawn to this location over a long period of time.

Identifying animals to species from relict proteins is a powerful new archaeological tool. Here the authors apply the method to answer questions relating to the Salish of west coast North America. Did they weave their blankets out of dog hair? The proteomic analysis shows that they did, interweaving it with goat, and that the woolly dog was increasingly superseded by sheep in the later nineteenth century.

Introduction
Ellis – van Creveld syndrome is an autosomal recessive disorder manifest by short-limb dwarfism, thoracic dystrophy, postaxial polydactyly, dysplastic nails and teeth, and an approximately 60% incidence of congenital malformations of the heart. Despite patients with Ellis – van Creveld syndrome being regarded as having a high surgical risk, few data are available regarding their outcomes following surgery for congenital malformations of the heart in the current era.
Materials and methodsIn this retrospective report, we summarise the clinical observations and outcomes of nine infants with Ellis – van Creveld syndrome who underwent surgery for congenital malformations of the heart between 2004 and 2009.
ResultsWe identified 15 patients with Ellis – van Creveld syndrome during the study period; 11 (73%) had haemodynamically significant congenital malformations of the heart warranting surgery. In two of these patients, surgery was not performed. Of the nine patients who underwent surgery, all of whom were infants, eight (89%) had various forms of an atrioventricular septal defect and one patient (11%) had hypoplastic left heart syndrome (mitral and aortic atresia). Among the nine patients who underwent surgery, four (44%) died at a median of 102 days with a range of 25–149 days post-operatively, mostly from respiratory failure. Respiratory morbidity was seen in all surviving patients, of whom three underwent tracheostomy.
ConclusionsSurgery for congenital malformations of the heart can be successful in infants with Ellis – van Creveld syndrome, but mortality is high and post-operative respiratory morbidity should be expected.

Samples of sewage, sewage sludge and sewage effluent from one or more of four sewage treatment plants were examined for the presence of Leptospira, Mycobacterium, Escherichia coli, Brucella abortus and Bacillus anthracis. Brucella abortus and Bacillus anthracis were not isolated. Eleven strains of E. coli potentially enteropathogenic for calves or piglets, eight pathogenic strains of Mycobacterium and one pathogenic Leptospira strain were isolated from 101, 189 and 189 samples respectively.
Sewage sludge is not considered to play a major part in the epidemiology of disease caused by these organisms.

Sixty three samples of dairy creamery effluent were examined for the presence of mycobacteria. Thirty two strains were isolated from 27 samples. These were classified as follows: M. fortuitum (13), M. peregrinum (6), M. gordonae (5), M. marianum (scrofulaceum) (4), unidentified (4).
Ten strains, representative of the groups isolated, were tested for their effect on experimental animals. None were pathogenic for guinea pigs or mice but a number produced a minimal amount of skin sensitization in guinea pigs to avian and mammalian tuberculin.

Ninety-eight samples of pig slurry from 54 farms were examined for the presence of salmonellas, porcine enteropathogenic strains of haemolytic Escherichia coli and mycobacteria. Salmonellas were isolated from 12 farms (22 %) and enteropathogenic E. coli from 13 farms (24%). Pathogenic mycobacteria were not isolated.
Salmonellas were isolated from 7 of 16 farms (44%) stocked with ‘minimal disease’ pigs compared with only 5 of 38 farms (13%) stocked with conventionally reared pigs. Conversely enteropathogenic coliforms were isolated from 3 of 16 farms (19%) stocked with ‘minimal disease’ pigs compared with 10 of 38 farms (26%) stocked with conventionally reared pigs.

Zinc Oxide (ZnO) is actively investigated for hybrid organic inorganic device applications. The interface greatly influences the electronic properties of these devices. Molecular surface modification of ZnO is being investigated for its potential to control the alignment of energy levels, charge transfer, as well as, interfacial chemical characteristics that influence device fabrication. In this study, octadecyltriethoxysilane (OTES) treatments of thin film ZnO produced by sol-gel decomposition were explored. The ZnO films were hydroxylated and then modified using OTES in solution. The condensation reaction of the OTES at the surface was promoted by the addition of a protoamine catalyst. Contact angle and infrared spectroscopy studies confirmed the surface modification and indicated that the coverage of the OTES was submonolayer. The modified ZnO films were reproducible and stable for long periods. The effects of the modification on subsequently spin-cast poly[3-hexylthiophene](P3HT) and on hybrid ZnO/P3HT organic solar cell performance are discussed.

Ceramics were subjected to organic residue analysis from two collections: a series of middle Copper Age (Bodrogkeresztúr) vessels hitherto known as ‘milk jugs’, curated in the Magyar Nemzeti Múzeum, Budapest, and a collection of early Baden (Boleráz) vessels from the recently discovered settlement of Gyo”r-Szabadrét-domb, in western Hungary. The aim of the analyses was to establish whether or not these vessels, often associated with milk based on typological criteria, were actually used to process, store or serve dairy products. The results of the analyses revealed that no dairy products could be securely identified in the so-called ‘milk jugs’. Nevertheless dairy products were identified in other vessel types.
